If you have questions regarding coverage please call the back of your card or call our office to verify acceptance of the insurance plan prior to making an appointment.

Please bring your insurance card and prior authorization information with you to your appointment.

You may be responsible for the following charges at the time of your office visit:

• Coinsurance amounts (due at time of check-in)
• Charges for services that are not covered by your insurance plan
• Any services denied by insurance
• Cosmetic services
